肾盂积水两种不同手术路径的比较

Comparison of two different surgical approaches for hydronephrosis

目的 探讨小儿肾盂积水手术中不同的手术入路途径。方法 我院对 1996～ 1998年 12 7例先天性肾盂积水患儿进行了肾盂成形术或肾切除术等 ,采用上腹部横切口 ( 10 7例 )和腰背部斜切口 ( 2 0例 )两种不同手术路径。结果 腰背部斜切口具有手术入路方便、术野浅表、损伤小 ,术后发热、伤口疼痛少等优点。结论 腰背部斜切口是一种安全、损伤小、操作方便的手术路径 ,主要适用于肾脏活检、肾盂成形及单纯肾切除术等。

Objective To compare two different surgical approaches for children undergoing pyeloplasty.Methods 127 children undergoing pyeloplasty or nephrectomy were reviewed retrospectively from 1996 to 1998 year. The flank approach was used in 107 cases and dorsal lumbar incision was in 20 cases.Results Patients undergoing pyeloplasty via dorsal lumbar incision tended to have less postoperative pain, excellent exposure and less damage with low complication rate compared to those treated with standard flank approaches.Conclusion The dorsal lumbar incision is a safe and efficacious approach suitable for pyeloplasty,simple nephrectomy or renal biopsy.
