摘要
目的 观察 2个不同剂量咪达唑仑对肿瘤患者全麻诱导前的循环、血氧饱和度及心电图的影响。方法 将 47例行全麻手术的肿瘤患者 ,随机分为A组及B组 ,麻醉诱导前分别静脉注射咪达唑仑 0 .10mg/kg及 0 .0 5mg/kg。记录用药前后的HR、Bp、SpO2 、S T段、T波及QTc的变化 ,并进行统计学处理。结果 2组患者的年龄、性别、体重无显著性差异。 2组患者用药后血压均下降 ,与用药前比较均有非常显著性差异 (P <0 .0 1)。A组心率增快 ,A、B 2组的血氧饱和度均下降 ,与用药前比较均有显著性差异 (P <0 .0 5 )。 9例患者出现呼吸抑制 ,其中 4例舌后坠 ,血氧饱和度下降 (<90 % )。 2组患者心电图变化与用药前比较无显著性差异 (P >0 .0 5 )。结论 咪达唑仑用于全麻诱导前的肿瘤患者效果满意 ,它能解除患者紧张情绪 ,缓解情绪紧张造成的血压波动 ,对心电图无干扰 ,但应注意呼吸监测 ,以防缺氧。
Objective To evaluate the influence of two different doses of midazolam on Hemodynamics and Electrocardiogram before induction of anesthesia in patients with cancer.Methods Forty-seven ASA grade Ⅰ~Ⅱ,scheduled for general anesthesia,were randomly divided into two groups,Group A,Group B.Each received intravenous administration of 0.10 mg/kg or 0.05 mg/kg of midizolam before induction of anesthesia.HR,Bp,SpO 2 were measured at various times and S-T segment,wave T,Q-T interval were recorded.Results No significant difference was found between the two groups in age、sex and bodyweight.HR,Bp,SpO 2 decreased significantly after midazolam intravenation(P<0.05) and no significant change in Electrocardiogram was found in both groups.A few patients had respiration suppressed and respiration and circulation were smooth in most of them.Conclusion Midazolam intravenous administration before induction of anesthesia in patients with cancer is beneficial,hemodynamics became stable and free from nervous anxiety.It is necessary to monitor their respiration to protect from hypoxemia.
