原发性结直肠恶性淋巴瘤的诊治体会

Diagnosis and Treatment of the Primary Malignant Colorectal Lymphoma

目的 探讨结直肠恶性淋巴瘤的诊治方法。方法 对 18例结直肠恶性淋巴瘤的临床资料进行回顾性分析。结果 肿瘤好发部位在右半结肠 ,临床表现以腹部肿块、腹痛、体重下降以及血便、腹胀为主要特点 ,病理类型绝大多数为非霍奇金淋巴瘤(NHL ) ,组织学以弥漫型B细胞为主。本病少见 ,临床易误诊为癌 ,治疗宜采用以根治性手术切除为主的综合治疗。结论 诊断时应综合临床表现、影像学及内窥镜检查结果 ,以提高诊断符合率 ;治疗应采取根治性手术切除为主的综合治疗 ,预后较好。

Objective To search the effective methods of diagnosis and therapy for primary malignant colorectal lymphoma(PMCL).Methods The clinical data of 18 patients with PMCL were retrospectively analyzed.Results This disease mainly originated in the right semicolon.The clinical manifestations were characterized by abdominal mass, abdominal pain,weight loss,bloody stool,abdominal distension etc.The non-Hodgkin′s lymphoma(NHL) was major of pathological type,the diffuse B-lymphocyte lymphoma were main histological type.This disease was clinically rare,it was easily misdiagnosised as carcinoma.The methods of treatment should be multimodal including chemotherapy and radiotherapy after radical resection.Conclusion The diagnosis must be based on the results of clinical manifestations,tomography,endoscopy.In order to improve the long-term results,it was essential to treat the patients with multimodal therapy after radical resection.
