乳腺癌经高强度聚焦超声治疗后的病理变化

The pathological changes of breast cancer after HIFU treatment

目的 观察高强度聚焦超声 (HIFU)体外原位治疗乳腺癌后病理变化特点 ,判断 HIFU治疗乳腺癌的有效性和可行性。方法 乳腺癌病人 2 3例 ,术前 1周作 HIFU治疗。术后作病理、电镜检查。结果 HIFU治疗靶区能完全覆盖乳腺癌包块 ,靶区内组织凝固性坏死 ,坏死范围达乳腺癌包块外边 2 cm,靶区外无癌组织残留。经HIFU治疗后乳腺癌同侧腋窝淋巴结反应性增生明显 ,窦组织细胞明显增生。结论 HIFU能体外原位杀死乳腺癌组织 。

Objective To observe the pathological changes of breast cancer after treatment by high intensity focused ultrasound (HIFU) in situ and to evaluate the efficacy and feasibility of HIFU in the treatment of breast cancer.Methods Twenty three patients with breast cancer received HIFU treatment one week before modified radical mastectomy.Pathological examination and electromicroscopic observation were performed.Results The area of HIFU treatment coverd the whole mass of breast cancer,all tissues in the area were coagulative necrosis which reached 2 cm beyond the breast cancer mass.There was no cancer tissue remained out of the HIFU treatment area.After HIFU treatment,significant reactive proliferation on the axillary lymphatic nodes and significant proliferation of sinus histocytes were seen.Conclusion HIFU can kill breast cancer in situ,which brings possibility for noninvasive treatment of breast cancer without breast ablation.
