头颈部恶性肿瘤大剂量动脉灌注化疗的近期疗效观察

Observation on the short-term therapeutic effect of pumping vast dose drug into artery in head and neck carcinoma

目的 :为探讨大剂量 PDD+5 - Fu动脉灌注治疗头颈部恶性肿瘤的疗效。方法 :用大剂量 PDD+5 - Fu经颞浅动脉灌注 ,治疗头颈部恶性肿瘤 ,并用同一方案 ,作静脉滴注 ,进行了前瞻性随机对比研究。结果 :动脉灌注组完全缓解(CR)为 47.2 % ,部分缓解 (PR)为 5 2 .7%。静脉滴注组 CR为 2 1.2 % ,PR为 72 .7%。两组疗效及副作用有显著性差异。结论 :认为大剂量 PDD+5 - Fu动脉灌注比大剂量静脉滴注疗效高 ,副作用小 ,是晚期头颈癌病人缓解病情 ,延长病人生存期的辅助方法之一。

Objective: To discuss the short\|term therapeutic effect of pumping vast dose PDD+5\|Fu into artery to treat advanced carcinoma of head and neck. Methods: The patients were divided two groups. Group A (artery pumping patients, 36 cases) were 21 males and 15 females, ranging in age from 31 to 78 years old,which consisted of nasopharyngeal carcinoma in 16, maxillary carcinoma in 6, tonsil carcinoma in 5, tongue carcinoma in 6 and parotid carcinoma in 3. Group B (vein irrigation, 33 cases) were 20 males and 13 females, ranging in age from 27 to 69 years old, which consisted of nasopharyngeal carcinoma in 14, maxillary carcinoma in 7, tonsil carcinoma in 5,tongue carcinoma in 4, parotid carcinoma in 2 and lower lip carcinoma in 1. Vast dose PDD+5\|Fu was pumped in the artery of temporal superficial (group A) or vein irrigation (Group B). First therapy process was completed in 5 days, and after two weeks, the therapeutic effect was detected. Results: The CR(compelet remission) and PR(partial remission) were 47.22% (17/36) and 52.78% (19/36) respectively in group A, and 21.21% (7/33) and 72.73%(24/33) respectively in group B. There was significance difference of CR between group A and group B (\%P<0.05\%). Conclusion: Result suggested that the vast dose PDD+5\|Fu pumped into artery to treat the advanced carcinoma of head and neck had good effect in short\|term.
