精氨酸增强TPN与常规TPN对急性胰腺炎大鼠肠道细菌易位影响的比较研究

摘要 目的 比较精氨酸增强TPN与常规TPN对急性胰腺炎 (AP)大鼠肠道细菌易位及肠粘膜一氧化氮 (NO)的影响。方法 雄性SD大鼠 6 4只 ,随机分成 :1)对照组 (n =16 ) ;2 )AP组 (n =16 ) ;3)AP +TPN组 (TPNs组n =16 ) ;4)AP +TPN +精氨酸组 (TPNa组n =16 )。每组 8只大鼠 ,分别于建立急性胰腺炎模型后第 1及第 5天剖杀取材。检测肠粘膜NO含量、肠系膜淋巴结、肝脏、胰腺及门静脉血细菌易位率。结果 急性胰腺炎大鼠肠系膜淋巴结、肝脏、胰腺及门静脉血细菌易位率 1天和 5天时明显升高 ,与对照组比较差异显著 (P <0 .0 5 ;P <0 .0 1)。TPNa组 1天时肠系膜淋巴结细菌易位率降低 ,与TPNs组及AP组比较差异显著 (P <0 .0 5 ) ;5天时肝脏及门静脉血细菌易位率降低 ,与AP组比较差异显著 (P <0 .0 5 )。应用精氨酸增强TPN后肠粘膜NO含量明显升高 ,显著高于TPNs组及AP组 (P <0 .0 5 ;P <0 .0 1)。结论 急性胰腺炎大鼠肠道细菌易位率增加 ;精氨酸具有使NO含量升高 。 Objective To investigate the effect of arginine enriched TPN on gut barrier of rats with acute pancreatitis.Methods 64 SD rats were divided into four groups at random:the Control group(sham-operated,n=16);the AP group(n=16);the AP+TPN group (TPNs group,n=16);the AP+TPN+Arginine group(TPNa group,n=16).Every 8 rats in each group were killed on the first and fifth day after inducing acute pancreatitis respectively.The level of nitric oxide (NO)and bacterial translocation rates were measured mesenteric lymph nodes,Liver,pancreas and portal vein.Results Comparing with control group on the first and fifth day after inducing acute pancreatitis,the bacterial translocation rates of mesenteric lymphaden,liver,pancreas of AP group increased significantly(P<0.01,P<0.05);Comparing with TPNs group and AP group ,the bacterial translocation rates of mesenteric lymphaden in TPNa group decreased significantly on the first day(P<0.05).The level of protein in intestinal mucosa decreased and bacterial translocation rates in portal vein increased significantly on the first day(P<0.01).Compared with AP group,the bacterial translocation rates of liver and in portal vein in TPNa group decreased significantly on the fifth day(P<0.05).Compared with TPNs group and AP group,the level of NO in intestinal mucosa of TPNa group increased significantly(P<0.05;P<0.01).Conclusion Acute pancreatitis can increase bacterial translocation rates;Arginine enriched TPN has protective effect on gut barrier of rats with acute pancreatitis.
