摘要
目的 :探讨碱性成纤维细胞生长因子 (bFGF)在门静脉高压症断流术后食管中的表达和意义。方法 :门静脉部分缩窄制备肝前型门静脉高压症 (PHPH)大鼠模型 ,并设立假手术 (SO)对照 ;在术后 3周施行联合断流术 ,分别在断流术前及术后 1天、3天、5天、7天、14天、30天和 90天取材 ;用免疫组织化学Envision两步法检测bFGF和第八因子相关抗原 (FVIIIAg)在食管组织中的表达 ;对FVIIIAg阳性血管进行微血管密度计数 (MVD) ,对bFGF染色结果进行计算机图像定量分析。结果 :PHPH组大鼠在断流术前MVD较SO组显著增加 (P <0 .0 1) ,在断流术后 ,则较术前明显下降 (P <0 .0 1) ,但在术后 5天又开始增加 ,至术后 1月与断流术前已无明显差异 (P >0 .0 5 ) ;SO组在断流术后MVD未下降 ,而是在术后逐渐增加 ,第 3天达到高峰 (P <0 .0 1) ,以后基本维持在此水平。PHPH组在断流术前及术后 14天、1月 ,bFGF蛋白表达均明显高于SO组 (P <0 .0 1) ,表达高峰在术后 3天和 14天 ,到术后 1月仍显著高于术前水平 (P <0 .0 5 ) ,而SO组在断流术后 3天bFGF表达较术前明显升高 (P <0 .0 1) ,高峰出现在术后第 5天 ,以后即逐渐下降 ,到术后 1月与术前已无明显差异 (P >0 .0 5 )。结论 :联合断流术可显著促进食管中bFGF的表达 ,而后者对于食管?
Objective:To evaluate the effect of basic fibroblast growth factor (bFGF) expression in esophagus after devascularization in portal hypertensive rats.Methods:Pre hepatic portal hypertensive (PHPH) rats were induced by partial portal vein stenosis, and the sham operated (SO) rats were built simultaneously. After 3 weeks, all the rats were devascularized combined with splenectomy, and the lower esophagus was transected and reunited at the same time. Each of the two groups was divided into eight sub groups and sacrificed before operation and 1, 3, 5, 7, 14, 30, 90 days after combined devascularization respectively. We detected the expression of bFGF and factor Ⅷ related antigen (FⅧAg) with the method of immunohitochemistry. Envision two steps, counted the microvessel density (MVD) through the results of FⅧAg staining and analyzed the results of bFGF staining with the computed pathology image analysis system.Results:Compared with the SO rats, the MVD of PHPH rats increased significantly before devascularization (P<0.01), then decreased immediately after the operation (P<0.01), and increased gradually from the fifth day. Until one month after devascularization, no difference from the pre operation stage could be detected (P>0.05). As to the SO rats, the MVD increased step by step after operation, reached its peak on the third day and undulated at this level since then. The expression of bFGF before operation and 14 days, 1 month after devascularization in PHPH rats was significantly higher than in SO rats (P<0.01), and there were two pinnacles of bFGF experssion after operation : the third day and the fourteenth day respectively. Until one month after devascularization, the experssion level of bFGF in PHPH rats was still higher than pre operation stage (P<0.05). The expression of bFGF in SO rats increased significantly on the third day after operation (P<0.01), reached its peak on the fifth day, and decreased gradually from then on, until one month after devascularization, no difference could be seen compared with the pre operation stage (P>0.05).Conclusion:Increased expression of bFGF promoted by combined deascularization accounts for the increase of MVD in esophagus. The portal vein pressure may also has its effect but not proportionally, on the expression of bFGF. Consequently, the changes of portal vein pressure is an important, but not the only factor in the regeneration of esophagueal varices after devascularization in portal hypertension, and the increased expression of bFGF may act as a promoter of the process.
