内镜下局部注药加外照射治疗中晚期食管癌

Endoscopic injection and radiotherapy on advanced esophageal cancer

目的 探讨内镜下局部注药加外照射治疗中晚期食管癌的疗效。方法 经内镜下注射化疗药物：5－氟脲嘧啶250mg，丝裂霉素2mg，瘤体区域分点注射，每周1次，5～6周。放疗于注药后第2d进行，放射源为电子直线加速器6MV－X线，前后对穿及后缩野照射，每次2．0Gy4次／周，总剂量48～56Gy／24～28次，共6～7周，上述完成后为一疗程。 结果 显效16例 （35．6％），有效26例（57．8％），无效3例（6．7％），总有效率93．4％． 结论 内镜下局部注药加外照射治疗食管癌有协同增效作用，副作用少，近期疗效较好。

Objective To study the curative effect with endoscopic injecting chemotherapy drugs plus radiotherapy on advanced esophageal cancer． Methods Forty five cases were treated with endoscopic injecting． chemotherapy drugs 5－Fu 250mg MMC2mg which were injected in the tumour region． The treatment was given once every week for 5～6 weeks． After one day all cases were given radiotherapy． The conventional radiation method of two fields was adopted with 6－MV X－ray radiation with medical lineal accelerater 200cGy per fraction and four fractions per week with total doses of 48～56Gy for 24～30 fractions． The endoscopic inject and the radiotherapy coustituted a course of treatment． Results CR 16 cases(35．6％) PR 26 cases(57．8％) NC 3 cases(6．7％) total effective rates 93．4％． Conclusions Endoscopic injection and radiotherapy on advanced esophageal cancer can coordinate each other and promote curative effect which had little side effect and well curative effect in the near furture．
