颌面中部外伤并发间接性视神经损伤治疗体会

The characteristic and treatment of the injury of middle jaw complicated by indirect optic nerve injury.

目的 探讨颌面中部外伤并发间接性视神经损伤的特征及治疗。方法 15例患者在进行颌面外伤处理后均应用皮质类固醇和脱水剂等治疗 ,对其中 3例视神经受压的患者进行视神经管减压术。结果 15例患者治疗前无光感 5眼(2 9 41% ) ,治疗后减少至 2眼 (11 76 % ) ,光感～ 0 .0 4治疗前 7眼(41 18% )降至 3眼 (17 6 5 % ) ,视力 0 0 5～ 0 3治疗前 5眼(2 9 41% ) ,治疗后提高至 12眼 (70 5 9% )。结论 颌面中部外伤患者在治疗外伤时应及早发现并发间接性视神经损伤。

Objective Inquiring into the characteristic and treatment of the injury of middle jaw complicated by the indirect injury of optic nerve.Methods Cortical steroids and dehydrating agents have been used to 15 patients after the treatment of the injury of middle jaw,among which 3 severer cases whose optic nerve were compressed have received decompression operation.Results Among 15 patients,5 eyes (29.41%) having no light perception before the treatment and induced into 2 eyes (11.76%) after the treatment.7 eyes (41.18%) with the vision of 0.04 before the treatment and induced into 3 eyes (17.65%).5 eyes (29.41%) visual acuity eyesight was 0.05～0.3 before the treatment increased to 12 eyes (70.59%) after the treatment.Conclusions The complicated indirect injury optic nerve should be found out earlier during the treatment upon the patients with the injury of middle jaw.There are close relations between the prognoses and the earlier discover.
