摘要
目的 :观察不同营养支持途径对严重烧伤后大鼠肠粘液成分的影响。 方法 :采用 30 %体表面积 度烧伤大鼠模型 ,随机分成伤前对照 (C)、静脉营养 (PN)及肠道营养 (EN)三组。 EN和 PN组给予等氮、等热量和等体积的营养液。在此基础上检测肠粘液层厚度、粘液中己糖及唾液酸的含量 ,并采用粘液组化染色法和透射电镜对肠道杯状细胞的分泌状况进行观察。 结果 :烧伤后肠粘液层变薄 ,粘液中己糖和唾液酸的含量明显低于烧伤前。杯状细胞结构受损 ,粘液分泌量下降 ,酸性粘液明显减少 ,而中性粘液明显增多。两组相比 EN组大鼠杯状细胞结构较完整 ,肠粘液层厚度、粘液中己糖和唾液酸的含量均明显高于 PN组 ,且酸性粘液也明显增多。 结论 :严重烧伤后杯状细胞结构受损 ,合成粘液的能力下降 ,肠粘液层变薄 ,粘液成分改变。与静脉营养相比 ,肠道营养可减轻伤后杯状细胞受损程度 ,促进肠粘液合成 ,维护肠粘液化学成分的稳定。
Objectives:To observe the effect of different nutrition support route on changes in intestinal mucus components after burn injury. Methods:Wistar rats were inflicted with 30% total body surface area full thickness burns and randomly divided into three groups:namely burned and enteral nutrition (EN),burned and parenteral nutrition (PN),and normal control(C).In EN and PN group,rats were supplied with equal volume,calorie,nitrogen and other nutrients.The following indices including mucus layer depth,the content of hexose and acetylneuraminate were determined.Moreover, the information of synthesis and secretion of mucus from intestinal goblet cell was observed by mucus histochemistry and transmission electron microscopy (TME). Results:After burn injury,the structure of goblet cell was damaged and the ability of mucus secretion was decreased,mucus layer depth became thin,and contents of hexose and acetylneuraminate were remarked declined.Moreover,the acidity of mucus was decreased and neutral mucus was increased.Compared with PN group,the mucus layer depth was thicker and contents of hexose and acetylneuraminate were higher in EN group. Conclusions:After burn injury,the structure of goblet cell was damaged, the ability of goblet cells synthesis and secretion of mucus was declind,mucus layer depth become thin,and contents of hexose and acetylneuraminate were remarkedly decreased.Compared with parenteral nutrition,enteral nutrition is beneficial in minimizing goblet cell damage,promoting mucus synthesis and keeping intestinal mucus chemical component stable.
