非小细胞肺癌p53基因治疗的循证医学评价

Evidence-based Appraisal of p53 Gene Therapy for Non-Small Cell Lung Cancer

[目的]探讨 p53基因治疗非小细胞肺癌的临床研究证据。[方法]依照循证医学的文献检索策略检索MEDLINE ,应用系统评价专用软件包进行数据处理。[结果]欧、美两个多中心研究共9篇研究报告均不足以证实 p53基因治疗非小细胞肺癌的有效性。[结论]大量的体外、体内实验提示p53基因治疗对于非小细胞肺癌的治疗有效 ,但是目前尚缺乏临床证据证实其有效性 ,因此当前不推荐p53基因治疗的临床应用。加强 p53基因治疗的基础研究 ,特别是研究如何提高转基因效率 ,应该是今后工作的重点。开展 p53基因治疗非小细胞肺癌的大规模临床试验目前条件也不成熟。

To probe into the evidence of p53 gene therapy for non small cell lung cancer(NSCLC)from clinical trials.To search MEDLINE according to evidence based literature retrieval strategies,and to process date with the Cochrane Review Manager software(RevMan).Nine articles from two multi center clinical studies in Europe and America were insufficient to confirm the efficiency of p53 gene therapy for NSCLC.[Conclusion]Though many laboratory results from experiments in vitro have implied that p53 gene therapy may be effective for the treatment of NSCLC,the clinical evidence of efficiency is insufficiency.The clinical application of p53 gene therapy for NSCLC is not recommended nowadays. Enhancing the fundamental research of p53 gene therapy,especially the technique to increase the efficiency of gene transfer,should be emphasis in future work.In addition,the opportunity for large scale clinical trials of NSCLC p53 gene therapy has not been mature yet.
