自身免疫性甲状腺功能减低相关性肾病综合征病人甲状腺功能测定的意义

Significance of Measuring Thyroid Function Level in Patients with Autoimmune Hypothyroidism Related Nephretic Synarome

目的 探讨甲状腺功能 (FT3、FT4 、TSH)的变化在原发性肾病综合征和自身免疫性甲状腺功能减退相关性肾病综合征鉴别诊断中的作用。方法 31例病人分为原发性肾病综合征组 2 0例 ;自身免疫性甲状腺功能减退相关性肾病综合征组 11例 ;另选无肝肾功能损害、无内分泌功能紊乱、反复小便常规检查正常者 2 0例作为对照组 ;三组病例均测定血清中FT3、FT4 、TSH水平 ;通过甲状腺功能水平的变化来明确该检查在原发性肾病综合征和相关性肾病综合征中的鉴别诊断意义。结果 三组病例中FT3、FT4 、TSH水平均与年龄和性别无关(P >0 . 0 5 ) ;自身免疫性甲状腺功能减退相关性肾病综合征与正常对照组比较FT3、FT4 水平下降 ,TSH水平升高 ,二者有显著差异 (P <0 .0 5 ) ;原发性肾病综合征与正常对照组比较FT3、FT4 、TSH水平无明显差异 (P >0 .0 5 ) ;原发性肾病综合征组与自身免疫性甲状腺功能减退相关性肾病综合征组比较 ,后者FT3、FT4 水平下降 ,TSH水平明显升高 ,二者之间有明显差异 (P <0 .0 5 )。结论 FT3、FT4 水平下降 。

Objective To study the function of the change of thyroid function (FT 3, FT 4 and TSH) in primary Nephretic Syndrome and Autoimune Hypothyroidism related Nephretic Syndrome .Method 31 Patients were divided into primary Nephretic Syndrome group (PNS group,20 cases) and Autoimmune Hpothyroidism related Nephretic syndrome group (AHNS group, 11 cases) with 20 subjects with normal renal,liver and endocrine function as control group and the FT 3,FT 4 and TSH were respectively measured by immunoradiometry.The data was analyzed to evaluate the significance of Thyroid function in the differential diagnosis between the primary Nephretic Syndrome and Autoimmune Hypothyroidism related Nephretic Syndrome. Results The level of FT 3, FT 4 and FSH has no association with age and sex( P <0.05);were found in AHNS group lompared with the controlgroup,showing significant difference;while the FT 3,FT 4 and TSH showed no significant difference ( P >0.05) between PNS group and the control group;compared with PNS group,AHNS group showed lower FT 3 and FT 4 level while obvious higher TSH level ( P <0.05);showing significant difference. Conclusion FT 3, FT 4 and TSH level play an important role in the differential diagnosis between the Primary Nephretic Syndrome and Autoimmune Hypothyroidism related Nephretic Syndrome.
