支气管成形肺叶切除术治疗肺癌的体会

Experiences in Treatment of Lung Cancer with Bronchoplastic Lobectomy

我院1980.6～1989.7应用支气管成形肺叶切除术治疗肺癌10例,5例施行是袖式肺叶切除术,另5例施行的是连同肺叶的支气管楔形切除术。术后病理诊断为鳞癌6例,腺癌2例、小细胞肺癌2例。10例均取得较满意的近期疗效,无并发症和手术死亡。除2例相当晚期的小细胞肺癌患者分别于术后10个月和一年死亡外,其余的8例均生存,其中已逾5年者2例,最早的1例已达9年余。表明如病例选择适当,远期生存情况亦令人鼓舞,本文还对手术的安全性、适应症、技术操作和术后处理等问题进行了讨论。

Ten cases of bronchoplastic lobectomy for lung cancer were performed from June 1980 to July 1989. Five of them were operated by sleeve lobectomy and another five by wedge resection of bronchus together with resection of the lobe of the lung. They were pathologically diagnosed postoperatively as squamous cell carcinoma in six cases,adenocarcinoma in two cases. The immediate results of all these patients were excellent. There were no complication of bronchial fistula, stenosis, focal and thoracic infections, and no operative mortality. Out of the ten cases two very late cases of SCLC died ten months and one year respectively after ooperations. The other eight non—SCLC are still alive. Two of them have survived more than 5 years in which one of them has survived beyond 9 years after operation. It was shown that if the cases being appropriately chosen for bronehoplastic lobectomy, long term survival may be in prospect. The advantages of safety, indications, techniques of the operation and its postoperative management were discussed in this paper.
