摘要 目的 了解肺癌组织中p2 1waf1、p5 3和PCNA蛋白表达情况 ,研究肺组织良恶性增殖的不同机制。方法 运用免疫组织化学方法 ,检测 76例肺癌组织p2 1waf1、p5 3及PCNA的表达情况 ,分析三种蛋白表达与肺癌临床特征的关系 ,并与 5 9例肺良性疾病组织相关指标进行比较。结果  (1)肺癌组织p2 1waf1和p5 3蛋白表达阳性率分别为 75 %和47 37% ,PCNALI值为 0 44± 0 32 ,均显著高于肺良性疾病组织 (分别为 35 5 9%、3 39%和 0 0 9± 0 14)。 (2 )肺癌组织p2 1waf1、p5 3和PCNA蛋白表达与肺癌临床分期及细胞分化程度无关。各型肺癌之间p2 1waf1、p5 3蛋白表达也无明显差异。(3)肺鳞癌PCNALI值为 0 5 1± 0 32 ,明显高于小细胞肺癌(0 30± 0 36 )。 (4)肺癌组织p2 1waf1和p5 3蛋白表达与PCNA无关 ,p2 1waf1和p5 3蛋白表达之间也未发现相关性。而肺良性疾病组织p2 1waf1和p5 3蛋白表达与PCNA相关。结论 (1)肺癌组织p2 1waf1与p5 3蛋白表达明显上调。 (2 )肺癌细胞增殖程度很高。 (3)肺鳞癌细胞DNA损伤修复能力可能高于小细胞肺癌。 (4)肺良性疾病细胞增殖过程中 ,p2 1waf1、p5 3及PCNA三种蛋白有协调作用 ,而在肺癌细胞则否。 Objective To evaluate the expression of p21 waf1 , p53 and PCNA in lung cancer and to explore the difference between benign and malignant cellular proliferation.Methods 76 lung cancer cases and 59 cases with various chronic pulmonary disease were studied for the expression of p21 waf1 ,p53 and PCNA by immunohistochemistry. The relationship betweeen the expression of these proteins and the clinical characteristics of lung cancer were analyzed.Results (1) The positive expression rates of p21 waf1 and p53 in lung cancer were 75% and 47.37%, respectively. The labeling index (LI) of PCNA in lung cancer was 0.44±0.32. They were all significantly higher than those of chronic pulmonary diseases (35.59%,3.39% and 0.09±0.14, respectively).(2)Expression of p21 waf1 , p53 and PCNA was not related to tumor stage and tumor differentiation. There was no significant difference in the expression of p21 waf1 and p53 among the four major histological types of lung cancer. (3) The PCNA LI of squqmous carcinoma was 0.51±0.32, significantly higher than that of small cell lung cancer (0.30±0.36). (4) Expression of p21 waf1 or p53 had no relation to PCNA in lung cancer tissue . In chronic pulmonary diseases, the expression of p21 waf1 and p53 kept close links with PCNA.Conclusion (1)p21 waf1 and p53 are upregulated in lung cancer. (2) The degree of cellular proliferation in lung cancer is rather high. 93) The capacity of DNA damage repair in squamous lung cancer may be stronger than that in small cell lung cancer. (4)p21 waf1 , p53 and PCNA were cooperated with one another during the cellular proliferative process in chronic pulmonary diseases, but not in lung cancer.$$$$
