涎腺粘液表皮样癌c-erbB-2,bcl-2蛋白表达的临床病理意义

THE CLINICAL PATHOLOGIC SIGNIFICANCE OF THE EXPRESSION OF P53, bcl-2 and c-erbB-2 IN THE TISSUES FROM MUCOEPIDERMOID CARCINOMA

目的 :了解 c- erb B- 2和 bcl- 2蛋白在粘液表皮样癌中的分布 ,评价它们在粘液表皮样癌鉴别诊断及预后判断的意义。方法 :用免疫组织化学技术检测了 41例粘液表皮样癌和 10例正常涎腺组织的 c- erb B- 2和 bcl- 2蛋白表达。结果 :c- erb B- 2蛋白在正常涎腺中的分布特征为导管上皮全部阳性 (10 /10 ) ,而腺泡上皮仅有 1例阳性 (1/10 ) ;粘液表皮样癌的癌组织阳性率(85 .4% )低于正常涎腺的导管上皮 (10 0 .0 % ) ,但阳性程度强 ,明显高于正常涎腺的腺泡上皮 (10 .0 % ) ,P =0 .0 0 0。bcl- 2在粘液表皮样癌的阳性率有高于正常涎腺的趋向 ,但无统计学意义。结论 :在正常涎腺中 c- erb B- 2主要分布在导管上皮 ;粘液表皮样癌 c- erb B- 2的阳性率明显高于正常涎腺的腺泡上皮 ,可作为鉴别诊断的参考指标 ;未发现在粘液表皮样癌中 bcl- 2和 c- erb B- 2表达的预后意义。

Objective:To disclose the locations of the expression of c-erbB-2 and bcl-2 in the tissues from mucoepidermoid carcinoma (MEC) so as to evaluate the significances in the differential diagnosis and the prognose of MEC. Methods: The expression of c-erbB-2 and bcl-2 in the tissues from 41 cases of MEC and ten normal controls were detected by immunohistochemical technique. Results: The positive expression of c-erbB-2 in salivary gland ducts was detected in all of 10 normal controls while only one was positive in the salivary glands cells. The positive c-erbB-2 was detected in 85.4% of MEC cases, with stronger positive in the MEC than in normal salivary(P0.000).It seems that the positive expression of bcl-2 was more often detected in the MECs than did in the normal ones. Conclusion: The positiv expression of c-erbB-2 was more common in salivary gland ducts than in gland cells in the normal salivary glands. The positive c-erbB-2 expression was detected much more frequent in the tissues from MEC cases than that from the normal salivary glands cells, indicating the significance of differential diagnosis. No prognostic significance was found when the positive bcl-2 and c-erbB-2 were detected in the MEC cases.
