光动力疗法治疗食管贲门癌110例分析

Photodynamic therapy in treatment of esophagocardiac cancer——a report of 110 cases

目的 评价光动力疗法 (PDT)联合局部化疗治疗食管贲门癌的疗效。方法 应用PDT治疗食管贲门癌 110例 ,并对其中 70例联合应用内镜下局部注射化疗药物。对每一患者均先静脉滴注癌光啉 (PSD - 0 0 7) 3～ 5mg/kg ,于用药后 2 4、48或 72h分别以激光照射肿瘤局部。PDT联合局部化疗组除PDT治疗外 ,于每次光照前肿瘤局部注射 5 -FU 2 5 0～ 5 0 0mg或平阳霉素 8mg。结果 PDT联合局部化疗组的近期显效率高于单纯PDT组 (P<0 .0 5 )。部分病例随访 10～ 18个月 ,PDT联合局部化疗组的平均生存期较单纯PDT组延长 (P <0 .0 1)。结论PDT联合局部化疗治疗食管贲门癌安全而有效 。

Objective To study the effect of photodynamic therapy (PDT) combined with local chemotherapy in the treatment of esophagocadiac cancer. Methods Of the 110 patients with esophagocadiac cancer treated with PDT, 70 were treated by combined use of local chemotherapy under endoscope. The PDT was carried out 24, 48 or 72 min after intravenous injection of photocarcinorin (PSD-007, 3～5 mg/kg) by irradiating the entire tumor with laser.In the combined PDT and loacl chemotherapy group, 5-Fu 250～500 mg or pingyangmycin 8 mg was locally injected prior to the irradiation. Results The rate of significant remission in combined PDT and local chemotherapy group was significantly higher than that in the simple PDT group (P<0.05). Followed-up for 10-18 months showed the mean survival time of combined treatment group was longer than that of PDT group (P<0.01). Conclusion The results indicate that PDT combined with local chemotherapy is safe for esophagocardiac cancer, and its therapeutic effect is better than that of PDT alone.
