摘要
目的:探讨低分子葡聚糖对左心功能不全的血流动力学改变,以证实其可稀释血液、改善微循环,对左心功能不全的疗效。方法:本组20例左心功能不全患者,心功能皿级与Ⅳ级,其中冠心病10例,高血压心脏病6例,扩张性心肌病4例。静脉滴注低分子葡聚糖200 ml加压宁定25 mg,维持2小时。在滴注前及滴注后15分钟、30分钟、1小时及2小时进行血流动力学检查。结果:肺毛细血管楔压、肺血管阻力、肺动脉平均压降低,心排血量、计算心脏指数、在室作功指数增加,而心率、收缩压、舒张压在用药前后比较无明显统计学差异。结论:本研究表明静脉注射低分子葡聚糖加小剂量压宁定对左心功能不全有急性与明显的效益。
Objective: It was investigated the haemodynamic effect of low molecular dextran(LMD) in left heart dysfunction in due to certificate it's efficiency of haemodilution and improvement microcirculation to left heart dysfunction. Methods: There were 20 cases of left heart dysfunction of Ⅲ or Ⅳ heart function, there were 10 cases cororary heart disease, 6 cases of hypertensive heart disease, 4 cases of dilated car-diomyopathy in it. LMD 200 ml and urapidil 25 mg be maintained to 2 hours intravenous infusion. In all patients, the haemodynamic record was performed before and after 15 minutes, 30 minutes, 1 hour, 2 hours LMD infusion. Results: Pulmonary artery wedge pressure, pulmonary vascular resistance, mean pulmonary artery pressure were decreased. Cardiac output cardiac index, left ventricular stroke work index were increased after LMD infusion. There were no significant statistical difference in heart rate, systonic blood pressure and diastolic pressure, in before and after LMD infusion. Conclusion: The study demonstrated that intravenous LMD and low dose urapidil produced acute and significant haemodynamic benefit in patients with left heart dysfunction.
