断指再植患者不同时期心理特征及护理对策

Investigation and Nursing of Clinical Psychological Features of Patients with Digital Replantation

目的 了解断指再植患者不同时期的心理特征,提出相应护理对策。方法 应用症状自测量表(SCL－90)对68例断指再植成功患者,在术后3d、术后1个月及术后3个月分别进行测量,并与国内青年常模进行对照。结果 在术后3d,患者焦虑和恐惧因子分显著高于常模(P＜0．01);在术后1个月,躯体化、强迫、抑郁、敌对及精神病性等因子分显著高于常模(P ＜0．01);在术后3个月,强迫、人际关系、敌对和偏执等因子分高于常模(P＜0．01)。结论 断指再植患者术后不同时期心理特征不同,针对其特点对患者进行分期心理干预,有利于断指再植手指的成活及其功能的恢复。

Objective To investigate the clinical psychological features of amputated patients afterdigital replantation and to present relevant nursing measures. Methods Using SCL-90,the Mentalhealth states of 68 patients undergoing replantation were assessed on 3 days 1 month and 3 monthsafter digital replanation and were compared with those of control young domestic norm. Resultson day 3, the anxiety and phobia factor scores of the patients were significently higher thandomestic norm (P < 0.01 ). On 1 month, the somatixzation. obsessive compulsive depression.hostility and psychotism factors were higher than these of domestic norm. On 3 months, theobsessive compulsive .interpersonol sensitivity .hostility.paranoid ideation factors were higher thanthose of the domesticnorm. Conclusion The psychological status of replanted patients haddifferent characteristics in different phases,and the specific psychological nursing support would bebeneficial to improving replantation survival rate and functional recovery.
