Goldblatt大鼠心脏收缩功能的研究及Valsartan对其心肌收缩功能的影响

摘要 目的 :本文应用超声研究Goldblatt大鼠心脏收缩功能的改变 ,并应用AT1 受体拮抗剂Valsartan治疗 ,观察其对左室收缩功能的影响。方法 :34只SD大鼠 6~ 8周龄 ,分为Valsartan治疗组 (n =1 3 ,V组 ) ,非Valsartan治疗组 (n =1 1 ,NV组 )和假手术组 (n =1 0 ,S组 ) ,V组和NV组在左肾动脉置入内径为 0 3mm的银夹 (2klc) ,每周测 1次血压 ,每 2周测 1次超声心动图 ,并计算左室收缩功能 ,术后 2周V组给予Val sartan 30mg/ (kg·d)一次性灌胃治疗 ,S组和NV组每天用饮用水灌胃 ,共 1 0周。 结果 :(1 )各组大鼠治疗前一般情况和左室收缩功能 (EF、FS、mFS)无差异 ;(2 )与NV组比较 ,术后 1 0周V组尾动脉压降低 75 6mmHg ;术后 1 0周NV组相对室壁厚度、左室重量、左室重量指数明显增加 ,与V组和S组比较 ,有显著差异 ;NV组舒张期左室内径明显变小 ,与S组和V组比较有显著差异 ,而S组和V组比较无差异 ;(3)NV组的左室收缩功能明显下降 ,其中射血分数 (EF)、左室中层缩短率 (mFS)、实测 /预测mFS与V组和S组比较均有显著差异 ,而S组与V组比较无差异 ;但各组间的FS和预测mFS均无显著差异 ;(4)相关分析 ,各组大鼠实测 /预测mFS与左室腔舒张内径成正相关 (r=0 61 2 ,P <0 0 0 0 1 ) ;相对室壁厚度 (RWT)与经线室壁应力成? Objective:To study left ventricular performance by M-Mode echocardiograms and the effect of angiotension Ⅱ receptor antagioists Valsartan on left ventricular function in Goldblatt rats.Methods:34 males Spragne-Dawley rats (6~8 weeks old) were studied.The rats were divided into three groups namely valsartan treatment group (V group,n=13),nonvalsartan treatment group (NV group,n=11) and sham operation group (S group,n=10).The rats in both group V and NV were put on a silver clip (two kidneys,one clip)with 0 3 mm lument to the left renal artery.The rats of group S were performed sham operation.The systolic blood pressure and M-mode echocardiography were measured and performed respectively weekly and every other week.The left ventricular functions were calculated.Angiotensin Ⅱ receptor antagonist valsartan was given to group V at the dose of 30 mg/kg body weight per day,dissolved in part of the drinking water.Other rats which were not given valsartan were given water served as controls.Results:(1)There were no significant difference in the blood pressure and left ventricular functions (EF,FS,mFS) between any group before operation.(2)The blood pressure,relative wall thickness,left ventricular weight,left ventricular index in group NV were increased significantly after 10 weeks of hypertensin comparing with group V and group S (P<0 05);Left ventricular end-diastolic dimension in group NV was decreased significantly comparing with group V and group S (P<0 05).(3)Left ventricular functions in group NV (EF,mFS,observed/peadected mFS)decreased significantly comparing with group V and group S (P<0 05),but there was no significant difference in FS and predicted mFS in any groups.(4) Observed/predicted mFS was positively correlated with left ventricular and diastolic dimension (r=0 612,P<0 0001);Relative wall thickness was reverse to MESS (r=0 563,P<0 001).Conclusions:(1)Hypertension and systolic dysfunction can be developed in 2 klc rats.(2)mFS is better than FS in estimating the left ventricular performance.(3)Increasing of wall thinckness are both adjusting and damaging cardiac tissue.The damage is worse especially in concentric hypertrophic phases.(4)Valsartan can reduce the regression of LVH and improve the left ventricular function in 2 klc rats.
