咽淋巴环非何杰金淋巴瘤临床病理分析

Non-Hodgkin Lymphoma of Waldeyer's Ring——A Cliniopathologic study of 53 Cases

本文报道单纯放射治疗的咽淋巴环非何杰金淋巴瘤53例。多数为临床Ⅱ期(86.8%),均未经淋巴造影和剖腹探查。Ⅰ期和Ⅱ期疗后5年生存率分别为100%和42.86%、56.00%。疗后因腹腔播散和远处转移而死亡占总死亡数的25.00%和39.4%,因此建议采用上腹部预防照射和化疗。本文应用国际专家小组制定的工作方案分类,发现本组病例的90.6%为弥漫性和56.6%为中度恶性,后者以弥漫性大细胞性和混合大小细胞性类型为主。我们认为,此工作方案分类有助于临床和预后的比较。

Fifty-three patients with non-Hodgkin's lymphoma involvingWaldeyer's ring were treated with radiotherapy alone. Most ofthem (86.8%) had clinical stage ⅡE disease, without a staginglaparotomy and lymphangiogram. The 5 year survival rate is 100%for stage ⅠE and 42.86, 56.0% for stage ⅠE. The patients weredead with dissemination to upper abdominal region (25.0%) andmetastatic (39.11%) in other location. The prophylacticirradiation in the upper abdominal region and chemotherapy hadbeen strongly advised. A working formulation of NHL was utilized by morphologiccriteria, in order to facilitate clinical comparisons of case reportsand prognositic trial. The histologic pattern of the biopsies in 90.6%of the cases was diffuse and in 56.6% the cell type was intermedi-ate malignant grade, in which diffuse large cell type, mixed smalland large cell type would clearly be dominated.
