恶性淋巴瘤的形态定量研究及其在分类上的应用

随机抽取我院1968～1984年B淋巴细胞来源的弥漫型非何杰金淋巴瘤55例,应用英国AMS公司VIDSⅢ图象分析仪进行细胞形态定量测定,获得细胞的核面积、周径、直径、体积、长径、短轴与长轴之比及形状因子等参数,结果发现随着淋巴细胞的转化,核的多项指标均呈递增曲线,尤其细胞核体积在淋巴瘤各亚型之间有显著差别(P<0.01)。据此,我们试图将淋巴瘤细胞分为小、中、大三类:核体积<50μm^3为小淋巴细胞;50～140μm^3为中淋巴细胞;>140μm^3为大淋巴细胞。从恶性程度和细胞大小来看,本文这一分类法与我国及世界其他主要分类法是相吻合的。

Fifty-five cases of non-Hodgkin's lymphoma were collected.Through morphometrical study by means of image analysis apparatus,the surface area, sphere diameter, perimater and sphene volume ofnucleus, form factor etc were obtained. It was found that the curveof multiple index of nuclei was gradually rising with the lymphocy-tic transformation, especially the volume of nuclei showed signifi-cant difference between lymphoma subtypes (P<0.01). Therefore authord divied lymphocytes into 3 types, small cell, medium celland large cell. Small cell is smaller than 50 μm^3 in nuclear volume,medium cell is between 50μm^3～140μm_3 and large cell than 140. Inlight of the grading of malignancy in relation to cell size, authorwould like to propose a new classification of Non-Hodgkin'slymphoma.
