结直肠癌胃泌素水平及其受体表达的研究

The study of the gastrin and the expression of its receptor in colorectal cancer

目的 探讨结直肠癌组织和血清胃泌素 (gastrin ,GAS)变化及其受体 (gastrinreceptor ,GR)表达的意义。方法 运用放射免疫和免疫组化法检测大肠癌患者术前和术后血清、癌组织、癌旁和正常组织中GAS含量的变化及其GR的表达。结果 大肠癌患者血清GAS高于对照组 ,术后降低 ,均以Duke’sC、D期明显 (P <0 .0 5 )。癌组织中GAS含量显著高于癌旁和正常组织 (P <0 .0 1) ;癌组织和癌旁组织中的GR阳性表达率明显高于正常组织 (P <0 .0 5 ) ,而前两者之间无差异 (P >0 .0 5 )。结论 结直肠癌细胞能分泌GAS并具有GR ;术后GAS变化可作为判断结直肠癌患者手术疗效和预后的指标 ;癌旁组织中GR阳性率高 。

Objective To analyse the significances of the changes of gastrin in the serum and cancerous tissue and the expressions of gastrin receptor in the colorectal cancer patients. Methods With radioimmunoassay(RIA) and immunohistochemical techniques(IHC), the contents of preoperative and postoperative serum gastrin and the changes of gastrin in cancer tissue、cancer adjacent tissue 、normal tissue as well as the expressions of the gastrin receptor were investigated.Results The content of serum gastrin in the colorectal cancer patients is higher than that in the control groups.After operation,the content decreases especially significant in the stage of Duke's C,D( P <0.05).The content of gastrin is highest in the cancerous tissue ( P <0.01).The positive rate of the expression of the gastrin receptor in the cancerous and cancer adjacent tissues was higher than that in the normal tissue( P <0.05),whereas,the difference is not significant in cancerous and cancer adjacent tissue( P >0.05).Conclusion The colorectal cancer cell can secret gastrin accompanied with gastrin receptors.Hypergastrinemia denotes possible existence of colorectal cancer;The changes of the gastrin can be considered as a criterion of judging the operation effects and prognosis of patients;The surgical resected area in the colorectectomy should be extensive due to the high positivity of gastrin receptor in cancer adjacent tissues.
