改良经皮气管切开术的临床应用

Improved percutaneous dilational tracheostomy

目的 :探讨改良经皮扩张气管切开术 (IPDT)在急诊和ICU的应用价值。方法 :80例急诊、ICU需气管切开的急危重患者随即分为IPDT组及传统手术 (OT)组 ,对手术操作时间、并发症、安全性、置管时间及费用等方面进行比较。结果 :IPDT组操作时间 (7 8± 4 1)min ,与OT组 (2 6 4± 14 6 )min比较 ,P <0 0 1;OT组发生与操作相关性死亡 1例 ,而IPDT组无。结论 :改良PDT技术较传统的OT技术迅速、安全有效、创伤小、并发症少而轻 ,可广泛适用于各级医院 。

Aim:To explore the value of the improved percutaneous dilational tracheostomy (IPDT) in emergency and intensive case unit (ICU).Methods: A total of 80 critical patients who require tracheostomy were divided into IPDT group and operation tracheostomy (OT) group Compared the mean time, complication, security and cost in the 2 groups.Results:The meantime to perform IPDT was(7 8±4 1) min and the mean time to perform OT was (26.4±14.6) min,which had significant differences( P <0 01).1 patient died in OT group of the operation but none in the IPDT group.Conclusion: It is found that the IPDT group is more quickly, safe and easy, and the cost was less.It may be widely used in general hospital.
