主动脉窦瘤破裂的治疗体会(附4例报告)

TREATMENT OF RUPTURE IN AORTIC SINUSAL ANEURYSM——REPORT OF 4 CASES

主动脉窦瘤破裂4例中,急性发病2例,逐渐发病2例,心功能Ⅲ～Ⅳ级。2例未及时手术,随后死亡,2例手术治疗痊愈。手术是治疗此病唯一有效的方法。术时为防止心脏膨胀和保证心肌冷灌注效果,在并行循环和在主动脉根部灌注冷停跳液时,要堵住瘘口。就鉴别诊断、手术时机的掌握和心肌保护等问题介绍了体会和教训。

This paper reports 4 cases of rupture in the aortic sinusal aneurysm. 2 of them were attacked suddenly and the other 2 gradually, the cardiac function being Degree Ⅲ and Ⅳ. 2 of them were cured surgically and the other 2 died afterwards without timely operations. Operations were the only effective method to treat the disease. In order to prevent the heart from distending and ensure a good cardiac cold perfusion effect, the orifice of the fistula was blocked during parallel circulation and infusion of cardioplegic solution. Also discussed are the differential diagnosis, operative opportunity, myocardial protection and so on.
