脑积水围手术期处理对疗效的影响

Management of shunted hydrocephalus and its effect on the result before and after operation

目的 研究脑积水围手术期处理的内容和方法及对疗效的影响。方法 掌握好术前分流时机的选择、客观指征的判断、分流管阀门及压力类型的选择及分流部位的选择 ,术后作好分流泵的维护 ,病人活动时间及量的限定 ,抗生素的使用及判断分流管的工作状况 ,这些是提高分流手术成功率的关键措施。结果 78例病人 ,仅 5例出现并发症 ,疗效明显高于国内其他组数据资料 (P <0 0 0 5 )。结论 重视脑积水围手术期的处理是提高分流手术成功率、降低术后并发症的重要措施。

Objective To investigate the contents and methods in management before and after operation and to find the effects on the result of shunted hydrocephalus.Methods Before operation,it is important to select the suitable shunting opportunity,to judge the objective indications,to select suitable kinds of valve and pressure of shunt system and to select the right shunting place.After operation,more attention must be paid to the care of shunt system,limiting the patient′s activity and active time,to the reasonable use of antibiotics and to the judgment of the working status of shunt system.The above-mentioned procedures are the key steps to get the best results of shunted hydrocephalus.Results Out of 78 patients,only 5 patients suffered shunting complications.But this result was still better than that of another domestic report(P<0.005).Conclusion The important methods which can be used to get successful results and to reduce the complications of shunted hydrocephalus are that we pay more attentions to the management before and after operation.
