前列腺素E_1对心力衰竭患者心肾功能的影响

摘要 目的 :观察前列腺素E1(PGE1)对心肾功能不全的影响。方法 :使用Swan Ganze导管和生化检查方法对 14例心力衰竭合并慢性肾功能不全患者进行PGE1疗效观察。结果 :PGE1使SVRI,CO明显改善 ,并有量效、时效关系 ;使血浆BUN ,Cr下降 ,尿量及尿肌酐增加 ,中分子尿蛋白减少 ;SVRI,CO的改善率与尿血肌酐比的改善率密切相关。结论 :PGE1可改善心力衰竭合并慢性肾功能不全患者的心肾功能 ,并减低外周血管阻力 。 Objective:Chronic heart and renal failure are closely related in terms of pathophysiology When heart failure is complicated with renal failure, it is difficult to control clinically The purpose of this study is to observe the effect of PGE1 on chronic heart and renal failure Methods: The effects of PGE1 on hemodynamics and renal biochemical parameters were observed in 14 patients with heart and renal dysfunction by using Swan Ganze catheter and renal biochemical examination Results:SVRI, CO were improved markedly ( P <0 05)and explored dose response and phaes response cures after intervenuse PGE 1 Marked shifts were not found in HR*BP Declined LVSWI enhanced markedly ( P <0 05) All Parameters such as serum BUN, Cr, and urinary quantity were improved markedly ( P <0 05 respectively) except NAG and urine protein Medium molecular urine protein decreased ( P <0 05) Marked relationship between improving rate of U/B Cr and that of SVR ( r =-0 9) and CO ( r =0 7) was found Conclusions:Cardiac output and renal function could be improved and systolic pressure and resistance be declined in patients with heart and renal dysfunction The improvement of renal function related with that of hemodynamics in the aspect of U/B Cr elevating rate and SVR?CO improving rate
