肾动脉灌注化疗加栓塞术在治疗巨大肾肿瘤中的临床应用

Clinical Application of Chemotherapy of Renal Arterial Perfusion to Giant Renal Carcinoma plus Embolization

目的 研究肾动脉灌注化疗加栓塞术在治疗巨大肾肿瘤中的临床作用。方法 对 15例肾恶性肿瘤行肾动脉灌注化疗加栓塞术 ,肿瘤直径 10～ 2 2 .5cm ,化疗使用药物为丝裂霉素 2 0mg ,长春新碱 0 0 2mg/kg。应用明胶海绵加碘油施行肾动脉栓塞。结果 治疗后行根治性切除 12例 (80 % ) ,3例症状缓解。术中出血少 ,易剥离。手术时间缩短。结论 该法安全、有效 ,适用于巨大肾肿瘤治疗的辅助治疗。

Objective To study the role played by the clinical application of the chemotherapy of renal arterial perfusion to giant renal carcinoma plus embolization.Methods Chemotherapy of renal perfusion plus embolization was given to 15 cases of huge renal malignant tumors with the diameter ranging from 10～22.5 cm.For chemotherapy,mitomycin (MMC) 20 mg and vincristine(VCR)0.02 mg/kg were given.For whole embolization,gelatine sponge and ioldized oil were prescribed.Results After treatment,radical resection was performed on 12 cases (80%),while alleviation in symptoms was observed in the remaining 3 cases.During operaton,there occurred less bleeding,easy stripping,and short time for surgery.Conclusions Our approaches are highlighted by safety and effectiveness,which are suitable for gaint renal lesions when used as adjunct treatment methods.
