食管癌根治性放疗后局部复发的相关因素分析

The Correlative Factor Analysis of local Recurrence after Radical Radiotherapy of Esophageal Carcinoma

目的 探讨食管癌根治性放疗后局部复发的相关因素 ,寻找提高放射治疗食管癌疗效的途径。方法 对根治性放疗后局部复发再行手术治疗的 2 9例食管癌的临床病理特点进行分析。结果 Ⅰ度放疗反应 8例 ,Ⅱ度放疗反应 10例 ,Ⅲ度放疗反应 2例 ,7例表现为肿瘤局部管壁各层的放疗反应的不同步现象 ,2例癌细胞只有轻微的退化和变性。结论 放疗前结合食管超声内镜检查和CT扫描 ,充分了解肿瘤的外侵程度和分布情况 ,以肿瘤为中心定位 ,确保靶区的准确性和科学性是降低局部复发率、提高食管癌放疗治愈率的首要条件 。

Objective To discuss the correlative factor of local recurrence after radical radiotherapy of esophageal carcinoma and seek for a good way to the radiotherapy of esophageal carcinoma.Methods 29 patients who had local recurrence after radical radiotherapy were treated surgically,all the cases were analysed in clinical pathology.Results The Ⅰ-,Ⅱ-and Ⅲ-degree reaction of radiotherapy had 8,10 and 2 cases,7 cases were different in each layer of the esophageal wall,2 cases had slight degeneration and denaturation of cancer cell Conclusions To combine CT scanning with endoscopic ultrasonography(EUS) examination and understand infringement depth of tumor before radiotherapy,the tumor center should be taken on the simulator in order to assure the accuracy of range and improve cure rate of radiotherapy.
