静脉药瘾者丙型肝炎病毒感染的分子流行病学

Molecular epidemiology of HCV infection in intravenous drug abusers

目的调查重庆市静脉内药瘾者丙型肝炎病毒(HCV)不同基因型感染状况。方法静脉内药瘾者284例,其中男191例,女93例,年龄21岁～36岁,用酶标记免疫(ELISA)法检测HCV感染情况,用逆转录-聚合酶链反应(RT-PCR)与限制性长度多态性(RFLP)分型检测HCV RNA,并观察血清ALT,AST水平。结果重庆市静脉内药瘾者中,anti-HCV IgG阳性率为40.5％;HCV RNA阳性率为34.5％,其中HCV 1b型,2a型及1b/2a混合型感染率分别为33.7％,46.9％及19.4％,HCV基因型与血清ALT,AST水平变化无明显关系。结论重庆地区静脉毒瘾者HCV感染以2a型为主(46.9％),其次为1b型(33.7％),1b/2a混合型亦不少见(19.4％),HCV基因型与血清ALT和AST水平变化无明显关系。

AIM To investigate the prevalence of HCV infection in Intravenous drug users and molecular biological characteristics of HCV. METHODS The serum samples from 284 intravenous drug abusers with different levels of alanine aminortansferase ALT) were obtained, and anti-HCV lgG was detected by ELISA, HCV typed by RT-PCR and restriction fragment length polymorphism (RFLP) analysis of the 5 untranslated region. of the 284 subjects, 191 were men and 93 were women, aged 21-36 years. RESULTS Anti-HCV lgG was positive in 40.5% (115/284) and HCV-RNA in 34.5% (98/284) with intravenous drug abusers. The HCV 1b and HCV 2a ganotypes and the 1b/2a mixed ganotype accounted for 33.7%, 46.9% and 19.4% of HCV infection, respectively. The distribution of HCV ganotypes in the individuals with different levels of ALT was identical. CONCLUSION Infection of HCV 2a genotype was predominant in the intravenous drug users from Chongqlng followed by HCV lb and 1b/ 2a mixed ganotypes. The different ganotypes of HCV were not related to the change of serum ALT levels.
