摘要
目的 了解射频消融灶大小与功率及时间的关系、超声与大体病理测值的关系、消融灶与周围组织的关系。方法 新鲜牛肝 3只和牛肾 2只 ,消融灶分两组 :距肝表面 1cm和 4cm ;同时将肾脏置于肝表面 ,用不同的功率和时间实验。结果 10min时 ,2 0W ,40W ,6 0W ,80W的消融灶病理测值分别为 4.1cm ,3.8cm ,3.0cm ,3.3cm ,超声测值分别为 4.5cm ,4.0cm ,4.5cm ,4.0cm。 2 0min时 ,消融灶病理测值分别为 3.0cm ,4.1cm ,2 .8cm ,3.5cm ,超声测值分别为 4.3cm ,4.2cm ,3.4cm ,3.8cm。 30min时 ,消融灶病理测值分别为 3.5cm ,2 .9cm ,3.5cm ,3.4cm ,超声测值分别为 4.5cm ,4.0cm ,3.6cm ,4.1cm。超声和大体解剖测值相比 ,P <0 .0 1。消融灶和周围组织间有0 .3~ 0 .5cm的浅红色移行区。结论 射频消融灶的大小不随功率和时间的增加而增加 ,超声比大体病理测值大 ,两者差别有极显著性意义。消融病灶和周围组织间有移行区。
Objective To study the size of ablation lesions corresponding to the power and the time of the radio frequency,the extension of the ablation lesions to the adjacent tissue. Methods Select 3 pieces of calf liver and 2 kidneys,insert the multi electrodes into the parenchymal at the point of 1 cm and 4 cm beneath the surface of the liver, and insert the electrodes into the kidney which was placed on the liver. Results The size of the pathomorphology was 4.1 cm,3.8 cm,3.0 cm,3.3 cm for the 20W,40W,60W,80W at the 10 minutes duration,respectively The size of the US measurement was 4.5 cm,4.0 cm,4.5 cm,4.0 cm in diameter,respectively.So was the 3.0 cm,4.1 cm,2.8 cm,3.5 cm and 4.3 cm,4.2 cm,3.4 cm,3.8 cm,pathomorphology and US measurement,for the 20W,40W,60W,80W at the 20 minutes duration,respectively.So was the 3.5 cm,2.9 cm,3.5 cm,3.4 cm and 4.5 cm,4.0 cm,3.6 cm,4.1 cm,pathomorphology and US measurement,for the 20W,40W,60W,80W at the 30 minutes duration,respectively.There was more significant difference between US scanning and the pathomorphology, P <0.01.There was light red color zone about 0.3~0.5 cm width between the ablative lesion and the adjacent tissue. Conclusion The size of the ablative lesion is not proportioned to the power or duration of the ablation.The size is bigger in US scanning than that of the pathomorphology.There is statistically significant difference.There is a zone with light red color lining in the lesion and the adjacent tissue.The superficial ablative lesion will injury the neighboring tissues or organs.
