环扎加压术后黄斑表面膜形成和B超图像分析

Formation and ultrasonographic analyses of secondary premacular membrane after scleral buckling surgery

目的 探讨视网膜脱离环扎加压手术后继发性黄斑表面膜 (secondary premacular m embrane,SPMM)的发生机理。方法 34例 34眼 SPMM,分析其形成与手术前后超声图像。结果 B超下 ,SPMM表现为黄斑区无异常、粗糙感或带状回声 ,和临床检查并不一一对应。B超下不少后极部玻璃体后脱离眼临床检查可见黄斑区有玻璃体样物残留 ,而玻璃体在后极部有部分粘连和分离眼可形成从黄斑区旁发生的 SPMM。结论 局部残留的玻璃体或黄斑区附近特殊的解剖结构可能为 SPMM发生的重要条件。

Objectives To discuss the mechanism of secondary premacular membrane(SPMM) after scleral buckling surgery for retinal detachment.Methods The formation and ultrasonogrphic features around surgery of 34 SPMM were analyzed.Results SPMMs present 3 kinds of features of the B-scan ultrasonogram:no abnormality,rough and band-like echo;not corresponding to the clinical features.Some vitreous left on the macula can be clinically seen in ultrasonographic posterior vitreous detachment eyes.SPMM can develope from paramacular zone in some eyes having both vitreous adhesion and vitreous detachment at posterior zone.Conclusions Vitreous left on the local zone and special anatomical structure around the macular region may be the important factors of SPMM formation.
