Study on the Content of Serum HDL Subclasses in Obese Subjects

目的 研究肥胖者血清 HDL 亚类组成及相对百分含量的变化。方法 采用双向电泳 -免疫印迹检测法分析了 6 4例正常对照及 5 8例肥胖者血清 HDL 亚类组成及相对百分含量。结果 肥胖者血清中前 β1 -HDL、HDL3 c及 HDL3 b含量显著增加 (P<0 .0 0 1) ,而大颗粒的 HDL2 b含量显著减少 (P<0 .0 0 1)。肥胖者中男性前 β1 -HDL 及 HDL3 c含量显著高于女性 (P分别 <0 .0 1及 <0 .0 5 ) ,HDL2 b含量则显著低于女性 (P<0 .0 5 )。正常对照者中男性前 β1 - HDL 含量显著高于女性 (P<0 .0 5 ) ,HDL3 c及 HDL3 b含量较女性亦有所增加 ,而 HDL2 b含量较女性有所减少。肥胖者血清 TG浓度与前 β1 - HDL、HDL3 c及 HDL3 b水平呈显著正相关 (r=0 .382、P<0 .0 0 1,r=0 .2 32 ,P<0 .0 5及 r=0 .2 94、P<0 .0 1) ,而与 HDL2 b水平呈显著负相关 (r=- 0 .30 4,P<0 .0 1)。结论 肥胖者血清 HDL 颗粒直径呈变小趋势 ,提示其 HDL 成熟代谢过程受阻。而且无论肥胖者还是正常对照者中男性血清 HDL 颗粒直径均较女性为小 ,提示性别差异可能与血清 HDL 亚类组成有关。

Objective To investigate the change of the contents of serum HDL subclasses in obese subjects. Methods Contents of serum HDL subclasses in healthy controls( n =64) and obese subjects( n =58) were determined by two dimensional gel electrophoresis associated with immunodetection method. Results The pre β 1HDL, HDL 3c and HDL 3b contents increased significantly ( P<0 001) and the HDL 2b content decreased significantly ( P <0.001) in obese subjects as compared with those in healthy controls. In male obese subjects, the pre β 1 HDL and HDL 3c levels were significantly higher( P<0.01 and P <0.05, respectively) and HDL 2b level was significantly lower( P< 0.05), compared wih those in female obese subjects. In male control subjects, the level of pre β 1HDL was significantly higher than that in female subjects. In obese subjects, the concentrations of BMI and serum TG had positive correlation with those of pre β 1 HDL( r=0.382, P <0.001),HDL 3c ( r=0.232, P <0.05) and HDL 3b ( r=0.294, P <0.01) and negative correlation with that of HDL 2b ( r=-0.304, P <0.01). Conclusion The particle size of HDL in obese subjects showed a general shift toward smaller size, which indicated that the maturation of HDL could be abnormal in obese subjects. The particle size of HDL was found to be smaller in both obese and control male subjects than in female subjects, suggesting that sex seems to be related to the contents of serum HDL subclasses.
