手术负荷量引入医疗目标管理的研究与实践

Research and practice on the introduction of operating load calculation into medical target administration

通过医疗目标管理中遇到的对工作量考评问题进行研究与实践，即在手术操作项目基础上，进行大、中、小手术分类，并根据手术操作时间、操作难度、应急难度等强度和风险比例关系，提出了手术负荷量和手术负荷平衡核算的计算方法，最后引入医疗目标管理，将其结果与目标管理考评挂钩。手术负荷量的研究与实践至少解决了３个问题：即手术操作时间、操作难度、应急难度的科学结合；手术台负荷平衡核算；医疗目标考评的公平性。具有计算简便、应用可行、考评合理等特点。在调动医护人员积极性、提高医疗质量、满足病人需求等方面发挥了较好的作用，对医院开展高新技术、挖掘设备潜力、提高效率和效益有一定的现实意义。

A research and practice on the question of workload calculation in medical target administration is being conducted. On the basis of a specified category, an operation is divided into a big, medium or small one. Also taken into consideration are the time, difficulty, emergency and the proportional risk involved in an operation. The operating load with its equilibrium accounting is accordingly figured out and introduced into the medical target administration, the calculated figures and the target administration evaluations being coupled. Through the research and practice on operating load calculation, at least 3 questions are solved: the scientific combination of the time, difficulty and emergency; the equilibrium accounting of the operating load; and the fairness of medical target evaluations. Such an introduction has its advantages of simple calculation, practicality and reasonable evaluations. It plays a good role in giving medical personnel higher incentive to work, raising the medical quality and answering the need of the patients. It also has a practical meaning in introducing new and high tech, making full use of the equipments and leveling up hospital efficiency and profit.
