左侧结肠癌致急性肠梗阻38例手术治疗

Operative Management of 38 Cases of Acute Intestinal Obstruction Caused by Left Colon Cancer

目的 :研究左侧结肠癌致急性肠梗阻的外科治疗的相关问题。方法 :对 38例左侧结肠癌合并急性肠梗阻的患者采用不同的手术治疗方法。结果 :行肿瘤切除一期结肠吻合 2 2例 ,无吻合口瘘等严重的手术并发症 ;肿瘤切除近端结肠腹壁造口 12例 ,其中 4例 3个月后行造口还纳闭瘘 ;4例单纯剖腹探查的患者 ,2例因多脏器功能衰竭死于围手术期。结论 :积极做好术前准备 ,早期手术治疗是提高左侧结肠癌致急性肠梗阻治愈率的关键。

Objective To investigate the surgical management of the acute intestinal obstruction caused by the colon cancer.Methods 38 cases of left colon cancer complicated with acute intestinal obstruction were treated with different operative methods.Results 22 cases received tumor resection and Ⅰ stage colocolostomy without any complication.12 cases with tumor resection and abdominal colostomy in which 4 cases were reoperated of shut off the colostomy three months after the initial operation.4 cases were treated with simple surgery inspection in which 2 cases were died of multiple organ failure.Conclusions Prompt surgical preparation before operation and early surgical operation is the key point to prompt the survival rate of acute intestinal obstruction caused by left colon cancer.
