从CT表现及临床症状体征探讨腰椎间盘突出症推拿配合骨盆牵引疗法的适应症

THE EXPLORATION OF THE INCIDATIONS OF MANIPULATION OF MASSAGE IN COMBINATION WITH PELVIC TRACTION ON THE TREATMENT OF THE PROLAPSE OF LUMBAR INTERVERTEBRAL DISC ACCORDING TO CT FINGINGS AND CLINICAL SYMPTOMS AND SIGNS

通过对 70例腰椎间盘突出症患者的 CT表现和临床症状、体征分析 ,以及对应用推拿手法配合骨盆牵引疗法治疗 1个月后的疗效进行比较 ,认为该疗法的适应症是 :CT表现为腰椎间盘膨出 ;腰椎间盘中央突出 <5 mm;腋下型后外侧突出 <5 mm;主椎管较宽大的肩上型后外侧椎间盘突出症。病员主要症状体征 :1单纯腰痛者 ;2腰及下肢放射痛不超过膝关节者 ;3腰及下肢放射痛过膝关节 ,直腿抬高试验不出现强阳性 ,Millgram试验不出现强阳性者。

The CT findings, clinical symptoms and signs of 70 cases diagnosed Prolapse of Lumbar Intervertebral Disc (PLID) before and after the treatment of 1 month with massage manipulation in combination with pelvic traction were analyzed, the conclusion was: ⑴ the indications of massage manipulation in combination with pelvic traction on the treatment of PLID according to CT findings were: ①the bulge of the lumbar intervertebral disc;② the length of the prolapse of the center part of the lumbar intervertebral disc<5mm; ③the length of the posterolateral prolapse of the subaxillary type<5mm;④ the posterolateral prolapse of the broader suprashoulder type of the main vertebral canal; ⑵ the indications of massage manipulation in combination with pelvic traction on the treatment of PLID according to the main clinical symptoms and signs were: ①simple lumbar pain; ②the radiating pain of the lumbar and leg without reaching knee joint; ③the radiating pain of the lumbar and leg reaching knee joint without the intensive positive signs of Elevation Test of straight Leg and Millgram Test.
