肝脏实性小占位对比噪声比与病灶性质的相关性 被引量:1

Correlative Study Between Identifications and Contrast to Noise Ratio (CNR) of the Small Solid Hepatic Lesions

摘要 目的 研究对比噪声比 (CNR)在区分肝脏实质性小占位性质方面的价值。方法 随机选择经确诊的肝细胞癌、肝转移瘤和肝血管瘤病例共 67例 ( 10 1个病灶 )行前瞻性磁共振成像 ( 0 .5T) ,计算病灶的CNR值 ,并进行统计学分析。结果 在T1W图像及质子加权像上 ,CNR值对区分肝血管瘤和肝细胞癌、肝转移瘤无价值。在T2 W图像上 ,肝转移瘤与肝细胞癌的CNR值均显著小于血管瘤的CNR值 ,TE =60ms时CNR区分肝血管瘤和肝细胞癌的诊断正确率为 83 % ;TE =90ms时诊断正确率为 84 % ;TE =12 0ms时诊断正确率为 88% .结论 在T1W图像及质子加权像上 ,CNR对区分肝脏实质性占位的性质无明确价值 ,T2 W图像上测得的CNR值可用于区分肝血管瘤与肝细胞癌 ,且TE =12 0ms时诊断正确率最高。 Objective To determine the value of contrast to noise ratio (CNR) in characterizing the small solid hepatic lesions.Method Sixty seven patients with 101 solid hepatic lesions were prospectively performed magnetic resonance imaging at 0.5T.Calculated the contrast to noise ratios (CNR) of hepatocellular carcinomas,metastases and hemangiomas.Result In T 1 weighted imaging and proton density imaging,the values of CNR had no value for characterizing hepatic lesions.The values of CNR for hepatocellular carcinomas and metastases were significantly lower than those for hemangiomas in T 2 weighted imaging.Using CNR to distinguish hemangiomas from hepatocellular carcinomas,the accuracy was 83% at 60msTE,and was 84% at 90ms TE,and was 88% at 120msTE.Conclusion In T 1 weighted imaging and proton dentisy imaging,the contrast to noise ratio (CNR) had no value for characterizing hepatic lesions,the values of CNR can be used to distinguish hemangiomas from hepatocellular carcinomas in T 2 weighted imaging.The accuracy was highest at 120ms TE.
