急性心肌梗塞伴右束支阻滞的临床意义

The clinical significance of right bundle branch block in patients with acute myocardial infarction

目的 评价急性心肌梗塞 (AMI)患者并发右束支传导阻滞 (RBBB)的临床意义及预后。方法 将我院 1990～ 1999年收治的伴新出现的持续性 RBBB的 AMI患者共 15例作为观察组 ,以同期收治的无束支传导阻滞的 AMI患者共 2 0例作为对照组 ,分析两组的心肌梗塞范围 (MIS)、严重并发症及住院病死率。结果 观察组的 MIS大 ,严重室性心律失常发生率高 ,心功能受损严重 ,住院病死率高 ,与对照组相比有统计学差异。结论 伴随 AMI新出现的持续性

Objective To evaluate the clinical significance and prognostic value of the newly appeared persistent right bundle branch block (RBBB) in patients with acute myocardial infarction (AMI) Methods 15 patients with AMI and newly appeared persistent RBBB admitted in our hospital from 1990 to 1999 were studied as observed group The other 20 patients admitted in the same period with AMI but without bundle branch block were studied as the control group The myocardial infarction spectrum (MIS),severe complications and in hospital mortality were analyzed Results The observed group had higher MIS,more severe heart function damage,more severe ventricular arrhythmias and much higher in hospital mortality rate than that in control group Therer was significant difference between two groups Conclusion Newly appeared persistent RBBB with AMI is a mark of malignant clinical course and bad prognosis
