心脏肌钙蛋白T对老年不稳定型心绞痛患者心脏事件的预测

The predication of cardial troponin T in cardial events of elderly patients with unstable angina pectoris

目的 :探讨检测cTnT诊断老年UA患者心肌微小损伤及其预后的判断价值。方法 :对48例老年UA患者及42例健康老年人进行血清cTnT定量的测定 ,同时测定CK -MB ,并分组比较相关因素 ,观察住院6周内心脏事件的发生情况。结果 :老年UA组随Braunwald临床分级增高 ,血清cTnT值相应增高 ;老年UA组中 ,血清cTnT≥0.1ug/L者 ,心脏事件发生率为35.7 % ,高于血清cTnT<0.1ug/L者的心脏事件发生率5.8 %(P<0.05)。结论 :血清cTnT检测对老年UA患者危险分层有较好的判断价值 。

Objective: To investigate the significance of serum cardial troponin T(cTnT) determination in the diagnosis and short term prognosis of minor myocardial damage in elderly patients with unstable angina pectoris. Methods: Serum cTnT and creatine Kinase isoenzyme MB were measured in 48 cases of elderly patients with unstable anigna pectoris and 42 cases of elderly healthy subjects. Relative risk factors and occurrence of heart events within six weeks of hospitalization were analyzed. Results: In the elderly UA group, serum cTnT levels increased with increase of Braunwald classification class. The rate of cardiac events in elderly UA patients with serum cTnT≥ 0.1ug/L was much higher than that in those with cTnT<0.1ug/L(35.7% vs 5.8%,P<0.05).Conclusion: Detection of serum cTnT is of clinical value in risk stratification of elderly patients with UA. Positive cTnT indicates that elderly UA patients with minor myocardial damage had an unfavourable short-term prognosis.
