眶距增宽症治疗总结(64例分析)

RETROSPECTION OF SURGICAL CORRECTION IN ORBITAL HYPERTELORISM(64 CASES)

目的 矫正眶距增宽症所致的颅面部畸形。方法 用颅内外联合径路眶周截骨术、眼眶中下部 U形截骨、眶内侧壁内移手术治疗 64例眶距增宽症。结果 手术后颜面外形良好 ,除早期 1例死亡外 ,无其他严重并发症。手术时间缩减至 5～ 6h,术中估计失血减少至 2 0 %～ 3 0 % ,随访的二次手术率 3 3 %。结论 手术矫正眶距增宽症可获得满意的颜面外形效果。

Objective Retrospection of surgical correction of craniofacial deformities in orbital hypertelorism using intracranial and subcranial osteotomies. Methods In 64 cases we employed 3 surgical osteotomies to deal with various kinds of orbital hypertelorisms, i.e. intra and extra cranial approach for severe hypertelorism, U-shaped osteotomy (subcranial) for medium hypertelorism, and medial orbital wall osteotomy for low-grade one. Results Good craniofacial appearance were achieved in all our cases except 1 dead occurred in 1980. No other complications happened in this series. Average surgical duration have been decreased to 5～6 hours, and blood lose to 20～30% of each one's blood capacity. In third of follow-up cases, secondary orbital minor corrections were made in nearly 36^ of patients. Conclusion Surgical orbital osteotomies was the best choice to correct the craniofacial contour in orbital hypertelorism.
