慢性肝病患者TNF-α、IL-6、IL-8、HA、LN、PCⅢ水平测定的临床意义

Clinical Implication of the Determination of Serum TNF-α, IL-6, IL-8, HA, LN, PCⅢ in the Patients with Chronic Hepatitis

为探讨慢性肝病患者血清肿瘤坏死因子 ( TNF- α)、白细胞介素 6( IL- 6)、白细胞介素 8( IL- 8)、透明质酸 (HA)、层粘连蛋白 ( L N)、 型胶原 ( PC )含量变化及其在肝纤维化中的临床意义。取空腹静脉血 ,采用放射免疫法( RIA)测定 81例慢性肝病患者血清 TNF- α、IL- 6、IL- 8、HA、L N、PC 含量变化。发现慢性肝病患者血清 TNF- α、IL-6、 IL- 8水平明显升高 ,与对照组比较有显著性差异 ( P<0 .0 5或 P<0 .0 1)。TNF- α、IL- 6、IL- 8含量与 HA、L N、PC 含量正相关。与不同临床类型的慢性肝病患者血清胆红素含量平行测定二者成正相关。结果表明 :慢性肝病肝纤维化过程中 ,贮脂细胞的活化 ,胶原合成增加与 TNF- α、IL- 6、IL- 8细胞因子有关 ,细胞因子活性与血清胆红素一样可反映肝细胞损伤程度。

The changes in serum tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-8 (IL-8), hyaluronic acid (HA), laminin (LN) and procollagen type Ⅲ (PCⅢ) levels in the patients with viral hepatitis and the clinical implication in liver fibrosis were investigated. Serum TNF-α, IL-6, IL-8, HA, LN and PCⅢ levels were examined in 81 patients with viral hepatitis and 15 healthy volunteers by radio immnoassay (RIA). The levels of serum TNF-α, IL-6, IL-8 in the patients with chronic viral hepatic disease were significantly higher than in the control group (P<0 05 or P<0 01). The serum TNF-α, IL-6 and IL-8 levels were postively correlated with those of HA, LN, PCⅢ. The bilirubin level in the serum of the patients with chronic viral hepatic disease of different clinical types had a parallel relationship with the cytokine activities. It was indicated that TNF-α, IL-6, IL-8 are important cytokines in occurrence of viral hepatitis and in the development of liver fibrosis in viral hepatitis. The activities of cytokines can reflect the injured degree of liver cells as the bilirubin level in the serum.
