腰椎间盘突出症术后疗效CT分析

CT Analysis of lumbar disc herniation in postoperative cases

目的 :分析腰椎间盘突出症术后症状复发的原因 ,区分术后正常改变的CT表现和导致临床症状CT征象。方法 :收集 138例腰椎间盘术后患者的资料 ,进行术后CT征象与时间、疗效对照分析。分别分为两组 :(1)无症状组 90例 ,有症状组 48例 ;(2 )术后 3至 36个月 73例 ,术后37个月至 2 0年 6 5例。结果 :硬膜外纤维化、蛛网膜粘连、椎间盘突出、术后椎管狭窄、小关节失稳是术后症状再现或持续的主要原因。结论 :椎间盘术后症状再现的因素诸多 ,不能单一认为某种原因。

Objective:To analyse the CT postoperative changes cases of lumbar disc and evaluate the causes of the recurrent symptoms after laminectomy.To recognize and differentiate normal and abnormal changes with CT images.Methods:138 cases of lumbar intervertebral disc post-operative.The compariso between the results of CT and clinical data.Include postoperative time and postoperative vlinical symptoms.Dividing into 2 groupes:(1)a group without symptoms (90 cases),a group with symptoms(48 cases).(2)The postoperative time:3-36 months(73 cases),37 moths to 20 Years(65 cases).Results:The major CT findings after surgery included extradural fibrosis,revurrent disc hernation,lumbar spinal stenosis and adhesive arachnoiditis.Extradural fibrosis,revurrent disc hemiation,lumbar soinal stenosis and arachnoiditis,instability of the lumbar spine were considered the major causes of recurrent symptoms.Conclusion:The revurrent symptoms due to vombined factors were more serous than those of single factor.
