围手术期胰腺癌患者细胞免疫功能的表达

The Clinical Study of Perioperative Cellular Immune Function in Patients with Pancreatic Cancer

目的 :探讨围手术期胰腺癌患者细胞免疫功能的表达。方法 :用直接免疫荧光技术和流式细胞仪检测 36例胰腺癌患者介入治疗前后和手术前后外周血T淋巴细胞亚群CD3+ 、CD4+ 、CD8+ 和NK细胞的百分率。结果 :胰腺癌患者外周血CD3+ 、CD4+ 百分率和CD4+ /CD8+ 比值 ,与对照组相比明显低下 ,在介入治疗后 1周下降更显著 ,但在手术前 (介入后 3～ 4周 )有所恢复 ,在手术后出现再次下降 ;NK细胞比率变化无显著差别。结论 :胰腺癌患者的细胞免疫功能表达低下 ,介入化疗和手术创伤可加重抑制细胞免疫功能的表达 。

Objective: The cellular immune function was investigated before and after intra-arterial chemotherapy and operation in patients with pancreatic cancer. Methods: The positive percentage of T lymphocyte phenotype and NK cell in venous blood were examined dynamically by method of direct immunofluorescence and flow cytometry. Results: The percentage of CD3 +?CD4 + and the ratio of CD4 +/CD8 + in pancreatic cancer patients were much lower than those of the controls, they decreased remarkably after intra-arterial chemotherapy, only restored to primary level 3～4 weeks later; the above indexes decreased again after operation. There was no noticeable difference in the percentage of NK cell between the pancreatic cancer group and the control group. Conclusion: The cellular immune function in pancreatic cancer patients was depressed; Intra-arterial chemotherapy and surgical procedure could significantly inhibit the cellular immune function.
