摘要
①目的 探讨Ⅳ ,Ⅴ级颅内动脉瘤外科治疗的有效措施。②方法 Ⅳ ,Ⅴ级颅内动脉瘤病人 34例 ,全部采用Yasargil翼点入路显微手术夹闭动脉瘤 ,其中同时行去骨瓣减压术 7例 ,术前因脑室系统积血行脑室外引流术 6例。③结果 治愈 (正常工作 +部分工作 ) 15例 (44 % ) ,好转 (生活自理 ) 6例 (18% ) ,植物生存 2例 ,死亡 11例 (32 % )。④结论 Ⅳ ,Ⅴ级颅内动脉瘤应争取早期手术夹闭动脉瘤 ,手术技巧及术前。
Objective To study the effective measure of surgery for poor grade intracranial aneurysms.\ Methods Yasargil's approach was adopted for all 32 patients. External cranial decompression was used in 7 cases. A ventriculostomy was performed in 6 cases before operation. \ Results 15(44%) were cured, 6(18%) significantly improved , 2 lost consciousness and 11 patients died (32%). \ Conclusion Surgery should be done early for the poor grade intracranial aneurysms. Operative skills as well as pre-and postoperative treatment are important factors affecting the prognosis .
