伽玛刀治疗听神经瘤的临床研究

A Clinical study of acoustic neurinoma treated by Gamma Knife

目的 提高伽玛刀治疗听神经瘤的疗效 ,总结经验。方法 采用 OUR旋转式伽玛刀治疗 2 0例听神经瘤病人 ,观察术后病情变化。 11例病人于术后 8个月行 MR复查。结果 5例术后头痛加剧 ,经治疗 2～ 4个月后缓解 ;3例出现右侧面部麻木 ,6个月后症状消失或减轻 ;1例一过性轻度面瘫 ,4个月后好转。MRI复查 :肿瘤完全消失 1例 ,肿瘤体积缩小 >5 0 % 1例 ,>30 % 2例 ,>10 % 3例 ,肿瘤无变化 4例 ,其中 MR显示肿瘤中心出现低密度坏死灶 7例。本组无死亡 ,无特殊并发症。结论 伽玛刀治疗听神经瘤的疗效肯定 ,损伤小 。

Objective To summarize the experience of treatment of acoustic neurinoma by Gamma Knife Methods The preoperative condition of 20 patients with acoustic neruinoma treated by OUR rotated Gamma knife was observed 11 patients were followed with MR examination eight months after the treatment Results Headache aggravated in 5 patients postoperatively,and relieved after 2～4 months treatment Facial numbness was observed in 3 patients and alleviated or disappeared 6 months later Transient mild facial palsy was observed in 1 patient and recovered 4 months later.Postoperative MRI showed that the tumor disappeared in 1 patient,reduced in volume more than 50% in 1 patient,more than 30% in 2 patients,more than 10% in 3 patients,and no variation in 4 patients.No patient died and no serious complications were observed in the group.Conclusions Gamma Knife is effective for acoustic neurinoma with definite effect and relatively little injury and complication.
