目的 探讨 AIDS病患者常见的胸部影像表现及其病理机制。方法 归纳 1996～ 1999年在博茨瓦纳玛丽娜公主医院 (PMH) 5 2 7例 AIDS病患者的胸部影像资料 ,结合其临床资料和实验室检查结果 ,随访其治疗和结果。结果 肺部感染以肺结核 (PTB)多见 ,在 AIDS病早期其影像表现同陈旧性结核复燃 ;在后期同原发性肺结核表现一致。卡氏肺囊虫性肺炎 (PCP)典型影像表现为渗出性病变分布于肺门周围 ,向周围肺野发展呈明显融合趋势。细菌性肺炎的影像表现与正常人群大致相同 ,但进展快 ,程度重。淋巴瘤表现为非典型性纵隔淋巴结肿大 。

Objective To explore the common chest x ray manifestations and their pathological mechanism of AIDS patients.Methods 527 cases of patients with chest x ray manifestations were analysed together with their clinical and laboratory data.The patients treatments and prognoses were followed up.Results The chest infections are mainly of PTB and PCP.PTB appearances in the early stage of HIV infection,are similiar to those of reactivation PTB in the normal population,in the later stage,appearances becoming in keeping with primary PTB.The typical PCP appearances are of diffuse patchy consolidations in the perihilar regions and spreading peripherally,tending to be confluent in nature.Lymphoma presents with non specific mediastinal lymphadenopathy together with lymphadenopathy in other regions.
