肺炎链球菌诱导的实验性家兔鼻窦炎 被引量:7

摘要 目的 建立家兔实验性急性、慢性鼻窦炎模型 ,观察其细菌学和组织形态学变化 .方法 家兔 2 4只 ,在全麻下 ,行上颌窦造口术 ,阻塞窦口 ,留置棉绒 ,结合接种 10 1 1 cfu肺炎链球菌 0 .4 m L.术后 5 ,3 5 d行细菌学和病理形态学检验 .结果 急性鼻窦炎组家兔感染的鼻窦可见多量中性粒细胞、嗜酸性粒细胞渗出液 ,粘膜可见淋巴细胞、浆细胞浸润 ,粘膜水肿 ,部分腺上皮变性坏死 .慢性组家兔所有感染的鼻窦均表现出中度或重度的炎症 ,典型特征是纤毛损坏 ,脱屑 ,上皮组织化生 ,而且出现白细胞浸润 ,纤维化 ,骨膜增生和骨质退化 .造模的所有 16只家兔上颌窦细菌培养阳性率为 10 0 % ,而正常组细菌培养结果为阴性 .结论 成功建立了稳定可靠的家兔急性、慢性鼻窦炎模型 . AIM To establish an animal model for acute and chronic sinusitis in rabbits and to observe their mucosa and mucociliary change. METHODS Experimental sinusitis was induced in rabbits by blocking the osmium and occluding withcotton packs and inoculating 10 11 cfu streptococcus pneumoniae in 0.4 mL. The animals were examined histologically and bacteriologically after 5 and 35 days. RESULTS The actue model produced a sinus infection that was characterized by exudates of neutrophils and eosinophils, mucosal infiltration with lymphocytes and plasma cells, and epithelial degeneration. The infected sinuses of chronic model displayed signs of moderate or severe inflammation throughout the study period. Ciliary damage and desquamation, hyperplasia and metaplasia of the epithelium were characteristic features. Furthermore, heavy leukocyte cell infiltration, fibrosis, periosteal hyperplasia and bone degradation and formation were also frequently encountered. There was a 100% correlation rate between culture of specimen obitain from the maxillary sinus in all 16 animal of AS group and CS group. Normal animals showed no bacterial growth from the maxillary sinus. CONCLUSION Stable acute and chronic sinusitis model can be established by using the method mentioned above.
