贲门粘膜癌癌前病变与幽门螺杆菌感染关系的研究

Relationship between Precancerous Lesions of Gastric Cardia and Helico bacter Pylori Infection

目的:研究幽门螺杆菌(Helicobacter.pyloriH.pylori)感染与贲门癌癌前病变发生的关系。方法:对我国食管癌高发区河北省涉县偏店、固新两乡的201例当地居民进行上消化道内镜普查,在贲门区不同位点(贲门脊根部内镜下12点位和对侧内镜下6点位)取活检,活检组织经HE染色进行病理诊断,Giemsa染色找H.pylori。结果:河北省涉县居民H.pylori感染率61.2%;在H.pylori感染者中,贲门脊根部H.pylori阳性率97.3%,对侧6点位H.pylori阳性率64.3%,P<0.05。H.pylori在贲门粘膜慢性活动性胃炎组织中阳性检出率为88%。结论:H.pylori感染与河北省涉县居民贲门癌癌前病变的发生有密切关系。

Objective：The aim of this study was to investigate the relationship between precancerous lesio ns of gastric cardia cancer and helicobacter pylori infection．Methods ：A total of ２０１patients from Sexian，a high incidence area of esophageal carcinoma in Hebei Province of China，were examined by endosc opy．Biopsy specimen from diff erent sites of gastric cardia（clock １２and clock ６respectively in e ndoscopy ）were examined through normal pathological process．Helicobacter pylori were de tected in slides stained by Gi emsa method．Then statistical analy sis was perfo rmed．Result：The infection rate of helicobacter pylo ri in those patients from Sexian rose to ６１．２％；Of which helicobacter pylori infection，the infection rate of helicobacter pylo ri in the root of gastric cardia ridge（clock １２in endoscopy）was ９７．３％，significantly higher than that in clock ６in endosco py（６４．３％）．And in chronic active gastritis lesions，the infection rate of helicobacter pylori was ８８％．Conclusion：H．pylori infection is assumed as a n essential cause of precancerous lesions of cardiac cancer in the population of Sexian，Hebei Pr ovince，China，where has high incidence of esophageal cancer．
