甲状旁腺功能减退症16例临床分析

Clinical study on sixteen patients with hypoparatheroidism

目的报告16例甲状旁腺功能减退症(简称甲旁减),分析其分型、临床表现及诊断、治疗。方法对1980年至2000年住院的16例甲状旁腺功能减退症的病人进行回顾性分析。结果诊断特发性甲旁减6例、手术后甲旁减7例(甲状腺切除术后)、假性甲旁减3例。临床表现为抽搐、癫痫发作、精神症状、血钙降低、除假性甲旁减外甲状旁腺激素(PTH)降低。给予补钙治疗后症状缓解。结论甲旁减是由各种原因引起的甲状旁腺激素减少或结构异常、靶器官对其不反应的疾病。分特发性甲旁减、手术后甲旁减、假性甲旁减,共同表现为低钙血症,治疗原则是纠正低钙血症。

Objective To report the type,clinical manifestation,diagnosis and treatmemt of sixteen patients with hypoparathyroidism. Methods The clinical data of sixteen paitents with hypoparatheroidism admitted to our hospital from 1980 to 2000 were analyzed retrospec tively. Results 16 cases etiologically included 6 idiopathic,7 secondary and 3 pseudohypoparatheroidism.Tetany epilepsy,neuropsychiatrial symptoms,hypocalcemia were common clinical manifestation. There was also low level of parathormone(PTH)except pseudhypoparatheroidism.After treated with calcium,the manifestations disappeared. Conclusion Hypoparathyroidism is the disease whichcauses may include reduction of PTH,abnormal structure of parathyroid glands and the target organ no response to PTH.The common manifestation is hypocalcemia. The treatment is based on calcium therapy.
