不同胶体溶液对血液流变学影响的临床研究

摘要 目的:比较静脉输注羟乙基淀粉或人体白蛋白对病人血液流变学的影响。方法:选择ICU病人18例,随机分为A、B两组,分别输注10%羟乙基淀粉10±2.25ml/kg或20%人体白蛋白1±0.30ml/kg,于用药前、用药后即刻取静脉血(A组并在用药后24小时再取血)行血液流变学检查。结果:羟乙基淀粉可显著降低血液粘滞度、红细胞比积和红细胞聚集性,且其使前二者降低的显著性可维持到用后24小时。人体白蛋白使血浆粘度显著升高,而对其他各项指标无明显影响。结论:与人体白蛋白相比,羟乙基淀粉可明显改善血液流变学,改善微循环障碍,故适用于休克等有微循环障碍或有血液粘滞度增高隐患的危重病人。 Purpose:To compare the effects of hydroxyethyl starch on hemorrheology with human serum al-bumin. Methods: 18 patients of ICU were randomly divided into 2 groups( A and B) ,one was infused with 10% hydroxyethyl starth 10 ±2. 25ml/ kg 10% hydroxyethyl starth . Hemorrheologic data was measured. Results: Hydroxyethyl Starch could decrease blood viscosity、hematocrit and erythrocyte aggregation significantly. Hu-men Serum Albumin had no prominent effection on hemorrheology but could increased plasma viscosity signifi-cantly. Conclusion: Compared with humen serum albumin, hydroxyethyl starch can improve hemorrheology and microcirculation significantly, therefor it is indicated with those serious patients who have got shock or other dis-eases with microcirculation disturbance or the risk of blood viscosity increased.
