摘要 慢性肾功能不全患者普遍存在蛋白质 /氨基酸代谢异常。其可能机制和影响因素主要包括厌食、低蛋白饮食及对低蛋白饮食适应性的损伤、酸中毒、激素抵抗、能量代谢的异常、细胞因子的异常及支链氨基酸代谢异常等。目前营养和药物治疗包括低蛋白饮食并补充酮酸 /氨基酸制剂 ,使用影响蛋白质 /氨基酸代谢的药物生长激素、重组人促红细胞生成素和磷酸二酯酶抑制剂Pentoxifylline和Amrinone等。 Abnormal metabolism of protein and amino acids is frequently observed in patients withchronic renal failure.Anorexia,low protein intake,impaired adaptation to low nutrient intake,acidosis,hormonal abnormalities,abnormal metabolism of energy and branched chain amino acids,abnormalities of the cytokine system and other unidentified factors are the possible contributing mechanisms;Nutritional and drug treatment includes low protein diet plus amino acids/keto acids formulation and administration of growth hormone,recombinant human erythropoietin and phosphodiesterase inhibitor pentoxifylline and amrinone.
